As MRP, a physician will:
• Be responsible for ensuring coverage for the patients in their assignment, including but not limited to the following:
- Conducting rounds a minimum of weekly before noon on the preassigned day. NOTE: exceptions to the preassigned day are acceptable provided they are communicated in advance to the Director of Care and/or Clinical Leader.
- Responding to phone calls from unit staff to address issues between 0800 and 1700 hrs on regular business days, and provide a hand-off to the on-call physician where needed/appropriate.
- Working collaboratively with co-MRPs to avoid having more than two MRP’s from the same floor on vacation at the same time. The MRP is responsible for ensuring coverage for their patients during their absences from within the group of credentialed physicians at SJCCC. NOTE: when another physician is covering for the MRP, he/she must attend the facility to conduct rounds to ensure a physical presence on the preassigned rounds day for that MRP assignment.
- Accepting phone calls or on-site requests for patients in other MRP assignments requiring urgent attention, and where the assigned MRP is not present and/or available.
- Conducting additional onsite visits and telephone follow-ups as required.
- Visits should include appropriate assessment in the patient’s room, diagnosis, communicating diagnoses to patients and substitute decision-makers and discussing treatment options, ordering of necessary tests and procedures, obtaining consent for orders, reviewing test results and acting upon results.
- Communicate as needed with physicians and other allied health professionals to facilitate the care of the patients.
- Communicate with sending physicians/NPs for new admissions to address outstanding questions or missing information in the admission package.
- Work collaboratively with the other members of the interdisciplinary team as an equal partner in providing rehabilitative and/or supportive care to the patients.
- Participate in discharge planning and communication to the patient/family, in conjunction with the interdisciplinary team.
- Communicate effectively and collaborate with the patient and/or significant others, and interdisciplinary team in a timely fashion.
- Work collaboratively with the team to manage ethical issues related to the care of the patient.
- Attend patient/family meetings and team conferences as required, to be scheduled during regular weekly rounds, where possible.
- Designate patient as Alternate Level of Care (ALC) in consultation with the interdisciplinary team, and in accordance with the policies of the MoHLTC and the organization.
- Respond, in a timely fashion, to telephone calls/pages, requesting input on patient care.
- Complete requests for consultations with clear rationale and baseline information for the consultation.
- Complete documentation as appropriate, including but not limited to the following:
- Give the order to admit the patient to the facility and write admission orders for all patients admitted under his/her MRP assignment;
- Admission Note within 24 hours of patient admission that clearly sets out the reason for admission (unless History and Physical Exam and provisional diagnosis is recorded within this timeframe);
- History and Physical Exam within a maximum of 72 hours. If the Admission Note includes a history, physical exam and provisional diagnosis, it would satisfy this requirement;
- Ensure all orders written by self are signed, including telephone orders;
- Document need to transfer care; document acceptance of transfer of care (as required) and take accountability for the patient’s needs while MRP (even if not related to original reason for admission);
- Regular Progress Notes;
- Regular updating of the diagnosis, any complications and anticipated date of discharge;
- Patient/family meetings and team conferences, as needed;
- Discharge Order, based on the discharge date agreed upon by the interdisciplinary team and the patient’s progress towards their goals; and
- Discharge Summary, final note, and prescriptions for discharge – to be completed during the last visit prior to discharge.
