Position Overview:
We are seeking a dedicated and experienced Registered Respiratory Therapist OR Registered Nurse with good standing with their College to join our dynamic healthcare team. The successful candidate will work in primary care offices within the Cochrane area, providing integrated disease management that includes critical education, spirometry testing, and medical management support for patients with Chronic Obstructive Pulmonary Disease (COPD), asthma, and heart failure.

Key Responsibilities:
- Patient Education: Deliver individualized education to patients and their families with COPD, asthma, and heart failure on managing their conditions, medication adherence, and lifestyle changes.
- Spirometry Testing: Perform and interpret spirometry tests to assess lung function in patients, ensuring accurate results and appropriate follow-up.
- Medical Management Support: Assist in the development and implementation of care plans in collaboration with primary care providers to optimize patient outcomes.
- Patient Monitoring: Track patient progress, follow-up on treatment plans, and provide ongoing support for chronic disease management.
- Team Collaboration: Work closely with physicians, nurse practitioners, other healthcare professionals to coordinate care and ensure comprehensive patient management.
- Documentation: Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational standards.
- Patient Advocacy: Act as a resource for patients and families, addressing concerns and advocating for the best possible care.
- Research: Participate in team research projects.
