Respiratory Therapist - Elliot Lake Family Health Team
The ELFHT provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered and unattached population of patients in the local community and surrounding area. The ELFHT employs an interprofessional team of health care providers who work in a collaborative model of care.
The ELFHT is currently seeking a 1.0 FTE Respiratory Therapist (37.5 hrs/week) to join its growing team of health care professionals. The position is for 5 days a week.
As a member of the multidisciplinary team, the Registered Respiratory Therapist/Certified Respiratory Educator (RRT/CRE) role is to provide assessment of lung health and thorough care and support to patients and their families or caregivers including assistance navigating community resources.
The RRT/CRE fosters a collaborative approach to lung health management in the community, identifies and participates in continuing education and research opportunities as appropriate to the goals of the program.
This position requires travel and potentially work at two FHT sites, so the successful applicant must be able to travel and hold a valid driver’s license.
QUALIFICATIONS
· Registered with the College of Respiratory Therapists of Ontario,
· Certified Respiratory Educator or in the process of completing CRE certification,
· Possess enthusiasm for working with patients across a broad spectrum of ages and challenges,
· Demonstrated experience in community and/or primary care in an interdisciplinary model or care,
· Demonstrated expertise in identifying and addressing the needs of people at risk for functional decline, assessment of daily living activities and falls risk,
· Creative and have realistic ideas about how to improve the lives of patients,
· Ability and desire to develop new skills and evolve with a continuous improvement focus,
· Leadership, organization, research, evaluation, time management, communication, critical thinking, and interpersonal skills,
· Ability to work independently and within a team environment,
· Proficiency with computers and electronic medical record systems.
KEY RESPONSIBLITIES
· Develop long term therapeutic relationships with patients, conduct assessments using recommended preventative interventions and risk assessment, create patient learning plans,
· Liaise with other team members to achieve optimal outcomes for the patient,
· Employ critical thinking, evidence-based medicine, and best practice as frameworks for practice,
· Document observations, conversations, interventions, and collaboration on patient care in the patent record,
· Participate in community health initiatives such as education and building community networks with other organizations involved in improving community health.
